Position Summary: 

The Project Coordinator is a full-time position that supports and collaborates with the local health department to assist families of children and youth with special health care needs (CYSHCN). The Coordinator provides information and referrals related to insurance, health services, and community-based resources. This grant-funded program works in partnership with the Department of Health (DOH) and the Regional Support Center (RSC) to develop resources, expand access to technical assistance, and identify barriers to health care and supportive services. The program also facilitates family information sessions, community engagement initiatives, and collaboration with partners to ensure culturally responsive and inclusive processes. In addition, the program supports successful transitions from youth to adulthood and provides families with accessible information, support meetings, and training workshops. 

Key Responsibilities: 

  • Collaborate with the Department of Health and the Regional Support Center (RSC) to develop and maintain a comprehensive resource list for Children and Youth with Special Health Care Needs (CYSHCN) and their families. 
  • Provide timely, accurate, and relevant information and referrals related to insurance, health services, and community resources. 
  • Support and participate in family information-gathering sessions and community engagement activities to increase program awareness. 
  • Work collaboratively with community partners, including medical providers, childcare providers, and school district representatives, to assist families in accessing available resources. 
  • Develop and update local resource listings for CYSHCN families in coordination with the RSC. 
  • Ensure all communication, outreach, referrals, and engagement strategies are culturally and linguistically responsive. 
  • Support youth and young adults with special health care needs in navigating the transition to adult life. 
  • Share information with families regarding support meetings, trainings, and workshops offered by community organizations and the RSC. 

Training, Knowledge, Skills, Experience: 

  • Contractual agreement to obtain a bachelor’s degree in a relevant field (e.g., public health, social work, or a related discipline) within two (2) years. 
  • Experience providing assistance and support to families, particularly those with children or youth with special health care needs. 
  • Knowledge of insurance systems, health services, and community resources related to special health care needs. 
  • Ability to collaborate effectively with the Department of Health (DOH) and Regional Support Centers. 
  • Strong communication and interpersonal skills, including experience facilitating family information sessions and engaging community stakeholders. 
  • Demonstrated understanding of culturally responsive practices and experience working with diverse populations. 
  • Familiarity with the transition process from youth to adult services for individuals with special health care needs. 
  • Experience organizing and supporting meetings, trainings, and workshops. 
  • Ability to work both independently and collaboratively within a grant-funded program environment. 

Education Requirements: 

  • The undersigned party agrees to obtain a bachelor’s degree within two (2) years from the effective date of this contract. Failure to meet this requirement may result in termination of the contract or other consequences as outlined herein. 

Working Conditions: 

  • Contract Requirement: The CYSHCN Coordinator is required to report to the office on days mutually agreed upon by the Coordinator and the employer. 
  • The Local Health Department (LHD) will participate in quarterly statewide webinars focused on promising practices, attend in-person regional CYSHCN meetings, support family information-gathering sessions, and participate in community engagement activities to promote program awareness. 

Materials & Equipment: 

  • Educational materials provided by the Department of Health (DOH) and the Regional Support Center. 
  • Purchase costs for all non-expendable, tangible personal property, including furniture and office equipment such as printers, copiers, desktop computers, and related items. Requests must include a description of the item, cost per unit, quantity, and total purchase cost. 
  • Certain smaller equipment purchases may be classified as supplies (e.g., fax machines). Confirm classification requirements with the Department.
